SJAI 2012 - Registration Paperwork
Note: Most of the text from
this page has been lifted from the 2007 Eastern's website. I have found
that the USA Ultimate staff is much better at explaining all of this
than I am. :)

Team registration must be completed by Friday, April 6th. The following
registration materials must ARRIVE (not
postmarked) at St. Johnsbury Academy and USA
Ultimate by Friday, April 6th. If your materials
have not arrived by Friday, April 6th then your team will be dropped
from the tournament and you will receive no refund. Seriously.
1. Create a roster online
using USA Ultimate's online rostering system [Note: This is NOT the
score reporter system]. The online rostering system can be accessed by
logging into the USA Ultimate
membership page. ALL rosters MUST be created online by a team
leader. Paper rosters WILL NOT be accepted. The one-time-event fee will
be an option. The tournament is listed in the online rostering system
as 2012S ST. JOHNSBURY ACADEMY INVITATIONAL. A

2. USA
Ultimate membership dues - All players must be current USA
Ultimate members for 2012. Youth dues are $29. Membership status may be
checked through the online rostering system. Membership dues should be
sent to the USA Ultimate, NOT St. Johnsbury Academy.
As an alternative to becoming a full member, all tournament
participants have the option of using the one-time-event registration
option which costs $5 per player. You can read about that options right
here. The one-time-event form is online right
here. Note: Players can create accounts and
complete the waiver online even if they will be paying the one-time
fee. The waiver form for one-time fee players is the same as the online
waiver.
3. Liability waivers - All players must have a signed
waiver on file for 2012. Waiver status may be checked through the
online rostering system. A player can sign their waiver by logging into
their USA
Ultimate membership page. Minors (<18 years) must have their
parent/guardian's signature. All signatures can be done ONLINE
through the membership page system.
4. Medical
authorization forms - All minor (<18 years) players
must submit a copy of a completed medical
authorization form to St. Johnsbury Academy, not
USA Ultimate. Copies must also be kept on site by the
chaperone. Please indicate on your roster which players are 18 or
older.
5. Chaperone
background check - Each attending team must have 2
chaperones who are at least 25 years old. IMPORTANT: All
teams must have have at least one chaperone who has submitted a NCSI background check to
USAU. Full chaperone information can be found online right here.

Summary:
1) Rosters are created online
in the St. Johbnsbury Academy event. Note: This is NOT the score reporter system.
2) Membership dues are sent to USA Ultimate. (You can pay these online.)
3) Liability waivers are completed online.
4) A copy of each player's medical authorization form
is sent to St. Johnsbury Academy.
5) At least one chaperone submits an NCSI
background check to USA Ultimate. That chaperone must be added to
the team's online roster.

Registration at the tournament will take place on Friday, May
19th at St. Johnsbury Academy from 6-10pm outside the Black Box in
conjunction with the Friday night dance. Tournament staff will be on
hand to check teams in and take care of any outstanding paperwork. All
teams should check in at registration if they are in town. All
paperwork issues MUST be taken
care of by Friday night or the team/players in question will not be
allowed to play. If all of your paperwork is complete then you
do not need to check in.
The captain's meeting will be held Saturday morning, May 19th one
hour before the first round at St. Johnsbury Academy for the
girls division, at Lydon Institute and the Caledonia County Fairgrounds
for the open divisions. Competition issues, tournament logistics, and
any last minute changes to pre-tournament info will be discussed. All
teams must have a captain present at the captains meeting, even if they
have a first round bye.
PLEASE NOTE: Any unfinished
registration paperwork must be taken care of at the Friday night
registration.
